The cartilage to be inserted during augmentation is harvested at the rib. For female patients, to decrease scar mark, the incision made to obtain the rib graft would be done at the inframammary fold.
The length of incision depends on which rib is harvested. Normally incision for the fifth and sixth rib is 2 to 4 cm in the middle of the inframammary fold.
For male patients, the incision is done directly over the preferred rib to facilitate dissection.

After obtaining the cartilage, the incision wound would be tested for any noticeable/possible air leak and skin closure is performed using dermal sutures, or Steri-Strips.

2. An incision is made (either open or closed).
3. Rib grafts are placed.
4. Nasal skin and tissue are repositioned.
5. The incision is closed.
