What is Colostomy?

Colostomy is a surgical procedure that involves an opening in the belly (abdominal wall) to treat various problems that cause malfunction of the colon, or diseases that may be affecting a part of the colon and needs to be removed.

A colostomy changes the way the body works to allow stool to pass. The location for colostomy stoma to be placed depends on which part of the colon is affected. The stoma, acting as the lining (mucosa) of the intestine, would look like the inside lining of the cheek. It may look large at first but will shrink within 6 to 8 weeks after surgery to its final size.

Types of Surgery

There are a few different types of colostomies, depending on the condition.

· Ascending - The procedure involves creating an opening from the ascending colon, on the right side of the abdomen. This is the least common type of colostomy.

· Transverse - This procedure may result in one or two openings created in the upper abdomen, middle, or right side. In the case of having two openings in the stoma (called as a double-barrel colostomy), one will be used to pass stool and the other will be used for passing mucus.

· Descending or sigmoid - In this procedure, the descending or sigmoid colon will be used to create a stoma, typically on the left lower abdomen. This is the most common type of colostomy surgery.

How It Works

Colostomy works by the stoma functioning as the opening in the skin where a pouching system will be attached, allowing feces to be collected.

Colostomy Procedure

1. A hospital gown will be provided
2. An intravenous therapy (IV) will be placed in the arm
3. General anesthesia will be given within the IV drip
4. Treatment site will be cleaned
5. An incision or a few small incisions (for laparoscopy) will be made in the abdomen
6. An area at the intestine will be cut for stoma and a ring will be implanted as the end of the intestine

Temporary Vs Permanent

Colostomy can be temporary or permanent.

· Temporary colostomy - this type of colostomy may be used when a part of the colon (usually at the lower section) needs to heal after trauma or surgery. After healing, the colostomy performed can be reversed, returning the bowel function to normal.

· Permanent colostomy - a permanent colostomy, sometimes known as an end colostomy, is necessary for a few conditions which include 15% of colon cancer cases. The procedure is commonly performed when the rectum needs to be removed due to disease or cancer and the remaining portion will then be used to create a stoma.

Why is Permanent Colostomy Needed?

Conditions in which a permanent colostomy is needed include:

· A blockage in the bowel system
· An injury
· Crohn’s disease, which is an autoimmune form of inflammatory bowel disease
· Colorectal cancer
· Colonic polyps, which is extra tissue growing inside the colon that may be cancer or may turn into cancer
· Diverticulitis, which occurs when small pouches in your digestive system, called diverticula, become infected or inflamed
· Imperforate anus or other birth defects
· Irritable bowel syndrome, which is a condition affecting the colon that causes diarrhea, bloating, constipation, and pain in the abdominal area

· Bowel prep is often required to clean out the colon 1 to 2 days before the surgery
· Fasting may be required
· Taking antibiotics (as prescribed by the surgeon) may be necessary to prevent infections

Colostomy Post Care

· Do not lift heavy objects for at least 4 weeks after the surgery
· Daily activities may have to be avoided for at least 4 weeks after the surgery
· Driving is not recommended for at least 4 weeks
· A diet advised by the hospital staff should be maintained
